For all other plans, services are provided on a private pay (out-of-network) basis at this time.
If you would like to use your out-of-network benefits, a superbill can be provided upon request for potential reimbursement through your insurance provider.
As a courtesy, we may verify your insurance benefits prior to your appointment. However, insurance coverage and benefits vary by plan, and patients are ultimately responsible for confirming their coverage, deductibles, copays, and out-of-pocket costs directly with their insurance provider.
